Why is it doing this?

to your first question.. I'm pretty sure that's how you're suppose to do it, or at least that's how I was taught to drive stick.. Slowly release the clutch and when it starts moving begin to add pressure onto the gas pedal.. and when you shift are you pressing your clutch in all the way?
